How to apply

Step 1: Fill in the online application form

Step 2 : Confirm your details which must include a valid email address

Step 3 : Pay now by credit card or choose to pay by cheque made payable to Ann and Arthur Adlington

Step 4: Print the confirmation form and send it with two passport sized photographs (and a cheque if you haven't paid by credit card) to:
